LOST YOUR COVID-19 VACCINATION CARD?

How to request a record of your COVID-19 vaccinations:  

You can either email the Virginia Immunization Information System (VIIS) help desk at vaccinerecord@vdh.virginia.gov or call the VIIS Help Desk at 1-866-375-9795 to request an “OFFICIAL RECORD” of  your COVID-19 vaccinations.  The Help Desk is staffed from 8:30 a.m. until 5:00 p.m., Monday through Friday.

You can also fill out an Immunization Record Request Form.  Within 24-48 hours, a VIIS representative will contact you.  Click on the link: https://www.vdh.virginia.gov/immunization/immunization-record-request-form/

(*Please note  VDH will provide you with an “OFFICIAL RECORD” of  your COVID-19 vaccinations which will be a  8 1/2″ x 11″ paper and not a card.)
